Forms

A form is any document that you need completed by your physician that gives details regarding your physical or mental health. Examples of some forms you may require completed are WSIB, Disability Tax Benefit, Work forms, School/ Training Forms, Insurance forms, and MOT Driver’s Physicals.

Almost all forms will need to be completed during an appointment so you and your physician can complete the form together. Forms not requiring a new examination may be completed through a telephone visit.

Please complete your portion of the form, to the best of your ability, prior to sending it to your physician.

The completion of many forms is not a service covered through OHIP as they are not medically essential and are often requests made by a 3rd party. When a 3rd party requesting information has not explicitly stated they will cover the cost, the patient is responsible for all fees associated with its completion.
